Community Health Improvement Plan

A Community Health Improvement Plan for Bergen County is currently underway.  As one of the lead organizations, along with the Bergen County Department of Health Services and the Public Health Partnership, we have convened  more than 50 community based leaders to identify key health issues facing our community and the resources needed to properly address them.
