Bergen County Releases Community Health Improvement Plan
The Partnership for Community Health, Inc. is a 501 (c)(3) organization, comprised of over 120 organizations and countless individuals who are committed to working together on health improvement projects throughout New Jersey. Since its start in 1994, the Partnership has had significant impact on the lives of our residents and workers. Our work continues.
Through our “Healthy Youth Network”, we will reach out to all our young residents to teach and most importantly ‘empower” them to make sound, healthy decisions. In collaboration with the students and faculty of primary schools, higher education and specialty schools, The Record media and other organizations, we will conduct educational seminars, provide materials and strengthen the knowledge of our youth to grow up to be healthier adults.
The focus of our “Adult Preventive Health Coalition” is on promoting healthier lifestyles from young adults to our seniors. The Partnership has been named the pilot organization for the “Mayor’s Wellness Campaign” ,a statewide initiative aimed at securing the endorsement and hands-on involvement of municipalities leaders in order create healthier communities. Whether it be safe walking and biking trials or sponsoring health fairs, the goal provide resources to promote a healthy community.
The goal of our “Cancer Coalition” is to bring together major decision makers in the county and expand educational activities through awareness of the cancer continuum: Prevention, early detection, treatment and survivorship. These activities will incorporate the physical, social, spiritual and wellness components of cancer care.
Our “Cultural Competency Task Force” strives to build an understanding community by increasing awareness of our commonalities, encouraging respect for our differences, and effecting systemic changes. Regardless of age, walk of life, economic status, race or religion, we have an abundance of cultural wealth to share with each other, and we will utilize our local and statewide resources to bring to the forefront the richness of our diverse community.
We have also established a “Converting the Uninsured” initiative. By collaborating with insurance companies, the Healthcare Institute of New Jersey and community organizations, we effectively outreach to our residents with health education and make health insurance, prescription medications and medical services accessible for individuals who are currently uninsured or underinsured.
In collaboration with Bergen Community College, we are building the first-ever “Northern New Jersey Disabilities Network”, with the purpose of creating an advocacy, education and service support network for people dealing with physical, mental and learning disabilities. This web-based resource will be geared to patients, family members, caregivers, providers and educators.
A Community Health Improvement Plan for Bergen County is currently underway. As one of the lead organizations, along with the Bergen County Department of Health Services and the Public Health Partnership, we have convened more than 50 community based leaders to identify key health issues facing our community and the resources needed to properly address them.We invite you to join us as we continue living our mission. The more organizations and individuals with whom we partner, the more lives we will touch. From newborn babies to senior adults, there is much work to be done.
